This Expression of Interest (EOI) is for general practices in the Central and Eastern Sydney PHN (CESPHN) region to participate in the Intellectual Disability Primary Care Inclusion Program. This program aims to enhance, support, and build capacity of primary care providers to effectively care for patients with intellectual disability and assist their families and carers.
This program is a comprehensive, incentivised program to support general practices to undertake in-practice training and complete Quality Improvement activities. The program offers financial support of $5000 to general practices to undertake the Program. Funding will be provided across two payments of $2,500 (excluding GST).
The training and quality improvement activities in this Program can also contribute to GP CPD requirements.
The timeframe for the delivery of activities to meet program objectives is 4 months from the date of MOA execution.
The Intellectual Disability Primary Care Inclusion Program is an activity of CESPHN’s Project GROW. Project GROW is a component of the National Roadmap for Improving the Health of People with Intellectual Disability and focuses on improving models of primary care for people with intellectual disability, their families, and carers.

Q. We are interested in joining the program for multiple locations within the CESPHN. We would like to know- if there is a minimum number of practice staff, GP or nurses that is required for those training sessions and QI activities?
A. There is no minimum requirement for participant numbers, as practice sizes vary. The program is available to the whole practice and we encourage all staff, GPs, nurses and allied health clinicians to participate.
If all practices are applying for the program, then each practice must submit a separate EOI for each to be eligible for the funding of $5000. The program will be delivered separately within each practice to the staff, GPs, nurses and allied health clinicians working there.
If you would like to combine the program across your practices, submit one EOI. Please note, only one funding amount will be paid in this instance.
Q. Does the practice require the presence of a practice nurse to undertake the Intellectual Disability Primary Care Inclusion Program.
A. There is no requirement for a practice to have a practice nurse, only that the practice has capacity to undertake the training and conduct the Quality Improvement activities.
