CESPHN has recently commissioned two neighbourhood centres in our region to establish Healthy Ageing Hubs to support and empower older people to live well and independently in the community. The Healthy Ageing Hubs are available in the Inner West, at Newtown Neighbourhood Centre, and in the Sutherland Shire, at Gymea Community Aid and Information Service.
The Healthy Ageing Hub offers guidance and practical assistance to older people to help them live independently and support health and wellbeing based on individual needs. This may include guidance on how to access My Aged Care and planning for older age.
The Healthy Ageing Advisor can connect people to local services and programs as required, which may include health, social, legal or housing and accommodation services, financial, social or mental health support, group exercise and social programs, or care finder services for people requiring specialist and intensive support with My Aged Care.
Both the Inner West and Sutherland Shire Hubs are available to serve older people, their carers and family members who live in the Central and Eastern Sydney PHN region. A referral is not required, but you can make a booking with the Healthy Ageing Advisory via the link below.
